Treating Rhinitis with Leukotriene Modifiers

Leukotrienes play a significant role in asthma attacks. These chemicals, found in the mast cells that line the airways of the lungs and nose, enhance mucus production, constrict the bronchial passages, and promote further inflammation of the respiratory lining by attracting additional inflammatory cells into the airways.

Leukotriene modifiers, such as montelukast (Singulair), approved for the treatment of asthma (see Chapter 12), are drugs that block leukotriene activity at the receptor site, thus decreasing the amount of mucus generated by exposure to allergens.

Leukotriene modifiers may be used to treat patients whose allergic rhinitis symptoms don’t respond solely to antihistamines. However, the FDA announced that it is requiring a black box warning (used for medications that carry serious safety risks) for montelukast (Singulair) because of the risk of neuropsychiatric events (mental side effects) associated with this drug. The warning advises healthcare providers to avoid prescribing montelukast for patients with mild allergic rhinitis.
